Keep business and clinical information separate
This website collects organizational marketing inquiries and client support requests. Do not enter a patient’s name, diagnosis, fertility history, treatment plan, donor details, medical record, clinical image or other protected health information. Do not upload or paste patient lists. This website is not a patient portal, medical appointment system or clinical support service.

What our privacy notice does—and does not—mean
Our Privacy Policy explains the current business website and CRM intake. It is not a HIPAA Notice of Privacy Practices issued by a treating provider, a business associate agreement or a claim that the form is approved for clinical information. A disclaimer or checkbox does not make an inappropriate data flow compliant.
HIPAA responsibilities depend on the parties, functions and information involved. An engagement requiring protected information must be assessed separately, with any necessary agreements and an approved system in place before data is shared. Do not infer permission to send clinical information from our use of healthcare terminology or a CRM platform.

Review tracking in its actual context
This website loads Google Tag Manager with analytics and advertising consent signals set to denied. Container-managed tags require their own consent and data-collection review. HighLevel-hosted business forms are a separate connection. Service interests are selected inside the contact form and included when the inquiry is submitted to the CRM. The Cookies & Browser Storage Policy explains these features and controls.
Healthcare websites require a specific assessment of tracking, disclosures and any protected information involved. HHS guidance on online tracking also notes a court decision vacating part of its public-webpage interpretation; it should not be read as a blanket rule that every visit to a healthcare-related page is PHI. Refer to the current HHS guidance and its court-order notice with qualified advisers.

If sensitive information was sent by mistake
Email hello@fertilerank.com with the approximate submission time, the address used and a request for privacy review. Do not resend or quote the sensitive material. We can locate the relevant record and assess containment, deletion and any further obligations. For medical assistance, use your treating organization’s approved channel.

This form connects to our HighLevel provider when it loads. Submitting sends your business contact details and request to FertileRank’s CRM for review and follow-up. Submit business information only—no patient details, medical records or passwords. Submitting does not enroll you in marketing emails or SMS.
