What this policy covers
This policy explains cookies and similar browser technologies used in connection with the FertileRank website. Cookies are small values sent with certain web requests; local storage is browser storage associated with a website origin. Both can remember choices. This policy supplements our Privacy Policy.

Browser restrictions can shorten storage or prevent it. The retired single analytics preference is not accepted as current permission. HighLevel form and hosting technology have separate operational storage described below. Clearing browser storage does not delete an inquiry already delivered to the CRM.

Embedded forms and hosting technology
Forms load from fertilerank-forms.vibepreview.app and submit to HighLevel’s CRM infrastructure. This connection allows the provider to receive request metadata. Its services may use session, security or other operational technology. Provider cookie names and lifetimes are not controlled by this website; the inventory above is not a complete inventory of provider-managed storage. See HighLevel’s policy for its separate platform practices.
Forms are enabled by default and load lazily without an extra open-form click. Service interests are selected directly in the form. The optional website analytics setting does not control this connection. Use the embedded-form control below or in Privacy preferences to stop forms loading on this browser. Blocking a form after it has loaded cannot undo data already received by the provider.
Website hosting and security services process operational requests separately from optional marketing analytics. Blocking all third-party resources or cookies may affect embedded forms. You can still read the site and contact us by email.

Analytics and advertising status
Google Tag Manager (GTM-P4HZQHTM) loads on every page and connects to Google. Analytics and advertising consent signals default to denied before it loads. The measurement release requires the matching explicit choice before its analytics or advertising tags fire. Analytics permission does not grant advertising permission. Ad personalization remains denied. Form fields and service selections are excluded from the website data layer; page URLs used for analytics omit query strings and fragments.
The requested no-JavaScript Tag Manager iframe still connects to Google when JavaScript is disabled. Preference controls require JavaScript; measurement tags are not configured to fire from that fallback. HighLevel operational form delivery is independent of optional Google measurement. Manage your choices
Use your browser’s privacy or site-data settings to remove other stored choices, including data on the separate form-provider domain. Clearing site data resets your saved choices and enables the default form-loading behavior on a later visit. These controls do not delete CRM records; use our privacy request process for those.
Contact
Business inquiries
This form connects to our HighLevel provider when it loads. Submitting sends your business contact details and request to FertileRank’s CRM for review and follow-up. Submit business information only—no patient details, medical records or passwords. Submitting does not enroll you in marketing emails or SMS.
